EMAIL & TEXT POLICY

Our clinic hopes to maintain efficient communication between patients, staff, and doctors. In order to facilitate this, we ask that you provide consent for us to communicate with you via email and/or text.

Emails and texts may include information regarding the following:

● Updates for yourself or your relatives receiving care at our clinic;

● Information about referrals made to specialists, including reminders and updates about appointments;

● Invoices, glasses prescriptions, or instructional documents for eye care products you have purchased;

● Information on contact lenses you have trialed or purchased;

● Updates and information about eye health and vision care, office announcements and promotions;

● Follow-up emails to discussions that we have had in-person or over the phone.

● Appointment reminders and confirmations;

● Intake questionnaires, and;

● Prompts to review our clinic and services.

Privacy Policy: Personal information including your name, address, phone number, and email address will be collected and stored securely on our clinic server. We will not release this information to anyone outside our clinic, unless otherwise required for government institutions and secure third parties. The purpose of collecting this information is to ensure we can communicate with patients and provide them with updates and information about their care, and to ensure that patients are able to communicate with us. All information collected through intake forms is safely secured according to Ontario’s Personal Health Information Act (PHIPA). You may withdraw your consent at any time by contacting our clinic via email at info@drjaclyn.ca, by phone at 416-929-9133, or by visiting our clinic. Please allow up to 3 business days for us to remove you from our mailing and text lists. If you have any questions or concerns regarding this policy, please speak with the front desk staff before providing your information.

OTTO POLICY

Otto is the contact lens ordering platform used by our clinic. Otto makes ordering lenses simple and accessible for our patients. The following policy is issued by Otto:

I hereby understand that Dr. Jaclyn Chang, Optometrist, may be required to share my personal health information and record with government authorities, public or private health care or insurance providers, ophthalmic labs and some other third parties, only on an as-needed basis, or as required by law;

I hereby understand and agree that my information may be stored locally on the server of Dr. Jaclyn Chang, Optometrist, but will also be stored off-site in a secure location for purposes of data back-up and cyber security by Otto;

I hereby agree to be contacted personally by Dr. Jaclyn Chang, Optometrist or on behalf thereof by Otto in order to receive appointment reminders and order confirmations, and;

I hereby agree to be contacted personally by Dr. Jaclyn Chang, Optometrist or on behalf thereof by Otto in order to receive information regarding offers, promotions and exclusive sales tailored specifically to my optical needs.
